The Enhanced Primary Care (EPC) program was introduced to provide more preventative care for Australians and improve the coordination of care for people with chronic conditions and complex care needs. The program provides a framework for a multidisciplinary approach to health care.
Medicare Rebates
The Medicare Rebate for patients referred by their GP under an Enhanced Primary Care Plan is $50.05 per consultation. Medicare rebates are only available for patients referred under an Enhanced Primary Care Plan by their GP. Some examples of chronic diseases that people on an Enhanced Primary Care Plan may have are listed below. If you can relate to any of the listed chronic diseases, you can approach your doctor with the forms provided below to discuss being put on an Enhanced Primary Care Plan. On an Enhanced Primary Care Plan, your doctor will discuss with you allied health professionals that will collaborate with your GP, to best care for your chronic disease. For example, if someone had depression, their plan may include the GP, a psychologist and an Exercise Physiologist. As another example, if a patient had diabetes, their plan may include the GP, a dietitian and an Exercise Physiologist.
Chronic Diseases
A ‘chronic’ disease is defined as one that has been or is likely to be present for 6 months or longer. Seventy percent of general practice visits are related to chronic disease. Physical inactivity is the second largest contributor towards chronic disease and illness, the first contributor being smoking. The minimum recommended amount of moderate physical activity per week is 150 minutes. Sixty-five percent of patients undertake less than this minimum recommendation.

* There is a choice of either home visits or gym visits. Clients choosing gym visits for the consultations, pay their own gym entry fee for each 1 hr consultation. Clients must be on an Enhanced Primary Care Plan to receive a Medicare Rebate.
What do I need to do?
The following forms need to be taken to and completed by your doctor. You then bring these completed forms with you to the first exercise session. A Medicare Rebate can only be received when these forms have been brought along with you to the first exercise session.
